I’m having an issue as well. I’m running Genesis with Executive Pro child theme. This is what I see in my sidebar for the plugin. No button to turn on or off the title, just text.

I have the box checked in the theme settings to leave the title off. It makes no difference if I have it checked or unchecked I still see no checkbox in the plugin.

The first time I had someone fill it out I had a duplicate. 2nd time someone filled it out it produced their contact 4 times. No clue what’s going on.
Getting DOUBLE entries and now QUADRUPLE entries when people use this form to submit info to me. Any ideas on how to fix it? LOVE this plug in – did a video on it for YouTube already
