Maybe there is some conflict with other plugins? Try to disable all plugins for a moment except ZodiacPress. I guess your PHP version is >5.4

Your form looks differently. Maybe you should download original files and upload them manually.

Thanks, it works now. I wish I had such plugin available directly on Orders page as I have some customers not registered as users.
