In cf form:
[select list “test1|Message1” “test2|Message2” “test3|Message3”]
[submit “SEND”]
in mail body:
list: [list]
more info: [_raw_list] 🙂
insert thic code in header.php file before menu code 🙂

Try use this plugin: http://ww.wp.xz.cn/plugins/configure-smtp/
Configure email who will send you a message form contact form. Maybe do you have problem at serwer with smtp.
