Everything is working fine after patching the following files with

I tried with only wp-members enabled and I still have the issue.
I have 91 users and registration is disabled, we create users and they use Lost password to define their password
I manually patched wp-members but the error still remains, I upgraded to 3.5.1 today and still have an issue which disappears when the plugin is disabled:
