It’s not in my spam folder but I believe I figured out the issue with your test forms. I replicated your test forms and did the following:
- I mapped the fields to a Leads Value. I selected “No Mapping” for all the fields and now the email notifications are going through.

I’ve checked the “Set Email Response” and I didn’t turn on the Toggle Email Confirmation.

Hi, I’m not sure how to check that. The domain is with goDaddy and hosting is with Hostgator. How would I check if my servers mail function is off? Thanks!
