There is some documentation for configuring Akismet for Multisite at https://akismet.com/support/general/auto-activating-and-configuring-akismet-for-wordpress/
I don’t believe we have a recommendation one way or the other. How you configure it is a decision you should make based on how you want to adminster your Multisite network.

I’m not quite sure what you’re asking? Are you asking if Akismet is integrated with your registration form? Are you using a WordPress website? Are you using a form builder plugin, or something else? We’ll need a bit more information in order to help you.
