I’ve turned off all plugins with the exception of:
• Authorize.Net Payment Gateway WooCommerce Addon
• Woocommerce
• Woocommerce Subscriptions

I’ve also turned on default theme (twentysixteen)

And am still experiencing the issue.

No custom code.

Thanks for the quick response. Removing that code did not fix the issue.
