I too have this question. Actually the issue is I need to post a note with one of the field options to explain it on the form. There is no way to insert a note that I can see and making the caption the note is limited to only allow 50 characters.
I have the same issue. Forms sumbit without validation even though the required field selections are set. Captcha also does not work. Seems to me that the plugin is not applying any validation javascript or there is a scripting conflict with another plugin that breaks all javascript in the process. It worked until a few weeks ago then stopped.
