I was able to figure it out. Turned out to be an issue with the theme itself, and not the plugin.

I’ve experienced the same thing, but the fields are not able to be edited. Actually, none of the fields are able to be edited. It’s almost as if the the text fields aren’t even there.
Any ideas?
