CloudFlare is a reverse proxy.
However, if he has mod_cloudflare installed properly, he should not have any problems with CloudFlare. If he did not have the mod installed properly, all the IPs would be CloudFlare’s IP.
This has to be a problem with this plugin or another plugin.

The new new 0.9.2.8 version fixed this specific problem, but introduced another blocking AJAX calls.
Try just deactivating the “minify” option all together and just use the caching options. I had to do that on a few of my websites.
Yes, you can run W3TC without “All In One SEO Pack” without any features being disabled. It is just that the author of W3TC recommends using that plugin. I would recommend using “WordPress SEO” instead.
