Hi, make sure you’ve kept the file names the same. This seems to be the problem in 99% of the cases 😉

Odd. Are you sure you have your plugin updated? It shouldn’t be adding any <div>s anywhere. At least not in the latest version.
This problem will be solved on next 1.0.2 update (no release date available). Keep an eye out for updates 😉

It shouldn’t be showing above every image – only when hovered. Try cleaning the cache.
Regarding the next update, I’m not quite sure yet. Depends on how busy we are during the next few days 🙂
