My problem turned out to be related to use of WP Cerber, in spite of having followed all of their directions that should have fixed it. Once I disabled that plugin my Forminator problem vanished.
I am having this exact same problem when using Safari. It was reported to me by someone on our site, and I was able to duplicate the error. I am on the latest version of the plugin.
