Please disregard the second part as I found the reason in the documentation.

Another thing to add is when I go under the entries for the form it shows that there are entries with the number beside “All” however it does not show any entries below that, where they should all be listed. Not sure if this makes any difference but the Advanced File Uploader is the only section of the form.
