I have uninstalled and re-installed the plugin and I have still the bug.
In fact I have the complete list above the header (with probably a standart layout), and the complete list repeated under the header with my layout…

I did it and it works.
But I have a curious layout after this change : a part of the past orders list appears above the site header, and another below…

1st question : I can’t see the seller’s details anywhere
2nd question : I have 6 pages for one order (with 6 items). Do I have to edit a new template to have only one pdf page ?

Trying withe Opera (the first error came with Firefox); I had this message :
