I found the issue. It was a conflict with the Conductor plugin that I have. I found those error in the apache error logs. Once I disabled those plugins, the sitemap worked. Will submit a ticket with the plugin developer.
I too have had issues with the cookie not being saved in the browser. I’ve tried Chrome and Firefox and have manually looked for those cookies to appear in a list of cookies and they do not.

In reply to: [Redirection] Unable to import htaccess file after updateI too can confirm that the 2.3.6 version import function is broken. I downgraded to 2.3.5 and I successfully imported 330+ redirects in just a few seconds. I marked this plugin as incompatible with WP 3.9.1.
I however, did not receive a fatal error, just a blank Redirection plugin page.

In reply to: [CSV Importer] Urgent: Characters that doesn't support CSV import?I too have come across this issue. I’ve also used musicman’s suggestion, which does work for the majority of issues.
It doesn’t fix a few things like Microsoft Word-specific characters like em-dashes, double spaces, and a few others. But, it is a start.
