Resolved!
Updated the permalink structure to /%postname%/ and the issue resolved itself.

I have isolated the issue further.
Viewing the form from the page above causes it to not work, but if you view the same page by its ID it works.

I have WordPress setup to use that page as the front page. Any thoughts on how I can resolve this then?

If only it were that easy.
I have removed all javascripts and the code is 100% validated.
So as the page stands, there is nothing running for javascript aside from Jquery and your form.
I noticed that you load the js in the footer, and also read somewhere you load jquery, jquery form, and cf7 js. However I only have jquery form and cf7 loading. Jquery is however loading in wp-head, version 1.3.2. Not sure if you removed jquery from yours?
