I am having the same issue. When I removed the checkbox items from the form, it works fine. Something is very much wrong there and I now have a very upset client.
I use a /dev subfolder every time. This is what made me assume it was a path issue. Happens every single time 🙁
You make a good point, though, so I’d be willing to bet that anyone that does a subdomain instead thinks I’m crazy 😛
