FWIW, over the past few days I was getting the same 500 errors at random times. Disabled this plugin and am no longer getting them. Errors were being generated on the admin and front end.
you ever find a solution? Recently installed Offload S3 Lite to move my images to S3, and that appears to be blowing out thumbnail info on this plugin. Disabling Offload and it works great.
