It turns out that this was the fault of the Watermark plugin: http://ww.wp.xz.cn/plugins/alti-watermark/
I had done all the htaccess fixes and checks and it was driving me nuts. The Watermark plugin was active *only* on the base site (not network activated) and it completely broke all images in all sub-sites.
I’m not sure if that can be fixed, but the reason for that is so that old styles don’t get cached, so it’s a lot more helpful for it to work that way.
