You are correct – it was a rogue Chrome extension (called Auto Quality for Youtube https://chrome.google.com/webstore/detail/auto-quality-for-youtube/iaddfgegjgjelgkanamleadckkpnjpjc?hl=en) that was preventing it from displaying. I should have tried it in another browser.
