I am running 3.4.2, just installed this plugin and have the same problem. In fact my previous title and description tags seem to be gone now.
I’m wondering if it relates to the Venture Child Theme? The plugin works on one of my other sites so I used those settings on my ‘venture’ site. Both sites are at WP 3.4.2.
