the icon is missing caused by TinyMCE version 4.0 by WordPress core. (Since plugin version 2.xx the icon already missing)
At this momment I’m still working out to upgrade into supported version (to show the button).
But, you can still using the shortcode manually without the button from editor. (The button only helper without typing shortcode manually).
Please check plugin shortcode instruction to using the shortcode.

I’m sorry about forcing video quality it’s not possible this time because YouTube doesn’t provide such parameter to do that.
YouTube automatically detect video size.
But I’ll try first to manipulate the video frame size. If this worked, I’ll bring it to the next plugin update.
I’ll let you know if the hack method available.

Custom theme won’t affecting this plugin on post editor.Did you try uninstall and reinstall the plugins?

As it’s name, this plugin do is just simple embed using default iframe from YouTube and doesn’t retreive anything else.And this plugin doesn’t provide any Schema Rich Snippets.

Are you sure not caused by antoher plugin?
Because when this plugin calculating ratio and processing number,
all processed by PHP in the backend. So will not affected by number format.If the problem caused when you change locale (by Translate plugin on your website),
so the problem is the Translate Plugin, not this plugin.
Because it change all number into number format, and can’t filter which number must be changed or not.Please contact the locale/translator plugin developer to fix this.
