I have had a look through the settings and it seems okay to me.
Have a play around with the W3 Cache settings, specifically turn off all but MaxCDN and Cloudflare.
Compare ‘full settings’ vs ‘MaxCDN and Cloudflare only’ via tools.pingdom.com
I notice that the MaxCDN and Cloudflare decreasing DNS look-up and therefore improving load time.
I have exactly the same combination of W3 Cache, MaxCDN and Cloudflare on my sites and all of the pages are now loading 5 times slower.

I am going to double check your settings with my and will get back once I verify them. This is a really frustrating issue!
