I started using the first option (?lang=en) but there seems to be a problem with internal links, the language is always reset to the default after using such a link.
On the qTranslate forum there are some fixes, but as far as I can see, they will only work if you have the “URL Modification Mode” option set to “Use Pre-Path Mode” (your option 2).
Does anybody know whether I can change this option during a project or is it once and for all selected and I have to start all over?

Yes indeed, to my surprise it got activated later and seems to work.
I am absolutely new to WP. Wanted to install qTranslate and got that fatal error message as well. Any suggestions? thanks in advance
