It may be that you are using the gravity forms default name input which consists of 2 fileds (name &surname)?
I seems that the problem occured when activating and/ors which made conflicts.

When using Dynamic Fields addon, do I have to place ##COOKIE:{wordpress_test_cookie}## (with my cookie name) in the Form Submission Field and check IsValue in the mapping tab?
