I have the same issue with the form.
@mailpoet / @wysija my plugin is updated and still getting the same error.
I also tried creating the form using Elementor Pro but the integration does not even allow me to map all of the fields. Is there an updat for this?

Thank you for your help.
I found the Content After Header field in the edit page > oceanwp settings > shortcode section. Not the in Appearance > Customize > Header > General. So was not able to follow your steps exactly so it didnt work for me. Not sure if you have any other ideas, but thank you for replying to me question.
