You removed all the sites and recreated them manually one by one ?
I’m facing the same issue / situtation but I don’t know when it broke. Maybe since the beginning. I’m not using WP Cleanup plugin. I’m using
- WP 3.9.1 japanese version (recently upgraded from 3.9.0 to 3.9.1)
- MultiSites enabled with 3 sites
- A few plugins: Akismet, Annual Archive, Attachments, Checkmail Validation for Contact Form 7, Contact Form 7, Hello Dolly, Really Simple CSV Importer, WordPress Importer, WP Multibyte Patch
- Customized themes
I tried some debug too but haven’t figure out what causes the issue.
Looking WPCleanup tickets, it looks like there are similar issue with WP3.9:
