I have been using the development version 1.03 from https://github.com/facebook/wordpress/zipball/master. It’s been running OK for a month or so. The version number in the development version code is incorrect – it still says 1.02 although it is 1.03, so I just changed that myself.
Never mind my above post – I had added a new subdomain but forgot to put that into the allowed app webspace. So I do not get the API 191 problem now.
I tried the 1.0.3 code listed in a posting above but the linking to account does not work. This was a new install to a website without this plugin (v1.0.2) previously installed. This is running WordPress v3.4.2.

Error Message: Invalid redirect_uri: Given URL is not allowed by the Application configuration.I have just installed FB plugin 1.0.2, the version that comes when installing from inside FB. I get the same problem of access token expiring when posting to a fan page, and FB plugin disables the fanpage feature. This seems intermittent and happens after only overnight (or perhaps hours). Having to re-enable it almost always, and then manually repost on my fan page defeats the purpose of the social plugin. Add my name to the request for a fix.
