I am the maintainer of the blacklist used by this plugin. Thanks for bringing this issue to my attention. I agree that the “-online.” term should be removed, as this is a rather generic word and would cause some overly aggressive comment filtering.
I’ll remove this term (and similar “online” terms) in an update to the blacklist this week.
As for the “Excluded Terms” list not working, I’ll have to leave that question to Andrew Norcross, who actually wrote the plugin itself.

I would contact the developer of that plugin to test things from their end. There have been several reports online regarding issues with the mod_security plugin and other WordPress plugins.

As the maintainer of the aforementioned blacklist, I have been discussing this issue with Dan for a number of months.
As part of my troubleshooting, I have tried cutting back the blacklist by thousands of lines at a time to see if there was a specific section of terms that might have been causing the issue.
Based on my testing, I believe that it has too do with the size of the blacklist itself and not any specific term or phrase included in the data.
I’m happy to perform additional tests if it helps solve this issue.

In reply to: [Block List Updater] Is this plugin still supported?I have been using this plugin under recent versions of WordPress, up to and including the current 4.7.1 release. I can report that it functions as promised and really doesn’t require any updating at this time.
By the way, I’m the creator of the Comment Blacklist for WordPress as mentioned.

Sorry for taking so long to reply to your original comment. I would be curious to know what content was contained in the comments which were being blocked. I have never used Akismet on a site where I already have the blacklist in place, so I’m wondering if that’d where some of the comment stomping is occurring.
As for editors and pre-approved commenters being exceptions to the blacklist, that’s a function of WordPress, not the plugin. Regardless of your user status, if something in the blacklist is matched, that comment is bombed.
Keep in mind that I am constantly optimizing and updating the master blacklist. It could very well be that there were terms in the July 2014 release of the blacklist which could have been commonly used phrases within the context of your sites.
If you’d be willing, I’d love to have to test out the blacklist again (with or without the help of the plugin) and pass along any false positives to me.

Unfortunately, your suggested fix does not work for me.
I have confirmed that the two lines in the generate_filename function have been changed. I also deactivated and reactivated the plugin, but the placeholders are stil being replaced with hyphens.

- Blacklist Auto Updater by Sergej Müller
- Comment Blacklist Manager by Andrew Norcross
