But the problem seems to happen after the sheet is moved to Team Drive. We tried to rename the sheet and tab and do it again, but it did not work for any forms.
After submission of the form, it is redirected to a blank page with a single line as shown below

We are still able to get the email confirmation but the info does not go to Google Sheet.
