To clarify, the only way I can get featured images to show is by manually moving them within blogs.dir/files to manually created year/month folders so they match with the url.

Yes, I followed all that. Bizarrely things started working last night so I guess I must have been a little impatient.
Only issue I am having is that some of the images are being broadcast to the network of sites but not older ones.
Very strange.
