I think I’ve found the problem – the URLs for Ad Provider B were separated by space, not by a comma. I dropped a comma in between the URLs – now it works!
Thanks again for the comprehensive and prompt replies!
PS: It seems there is no conflict when I whitelist or blacklist URLs, post ID and categories in one block, all at the same time.

Second that. @amazonlinkbuilder please confirm that the plugin has been discontinued or that updates are coming.
