Thanks Frank. I’m still stuck. I tried deactivating and deleting autoptimize to start fresh, cleared all cache, but still the form doesn’t render properly. Even without autoptoize when I use the ?ao_noptimize=1, the forms renders as expected.

Would you know why? Anything I can try?

You were correct Frank, I tried the ?ao_noptimize=1 and it renders it correctly. How can I continue to use Autoptimize and not have this problem?
