Someone else helped me.
I had to tick the “try to match terms to existing child Product Categories”.
To me this is really unintuitive.
First: the placement of this option – above “Enable Mapping…” – as if it was a separate feature.
Second: If I can point to a specific category, why calling it “Try to match…” – as it was some hit and miss action, with probability of success lower than 1.
But maybe there are some other reasons for this that I am unaware of.

as far as address issue – well, there is no issue – I checked the order details in woocommerce and the client filled two separate fields instead of one, so that is why it appeared as broken line. Thank you.
As far as displaying the (z VAT) suffix I already changed the product price suffix to (z VAT) but it still is not showing on invoice, even if I switch the order status to “pending payment” and recalculate/regenerate the invoice.
