I’m having the same issue with a client site.
The website is password protected, but you can use the password gjacobson to replicate the problem.
Go to http://www.divorcingadvice.com
Click Strategy Session
Go to the bottom of the page and click Yes, I’d like to get started
Click open deposit form and submit the form.
The redirect URL I placed in my dashboard is http://www.divorcingadvice.com/strategy-form
