The above code works. Just make sure any dropdown menus are placed before anything with fields in your colums.

I have a couple of different buttons, and was trying ninja forms earlier to see if that was the issue. The orange ‘send us an email’ buttons have the mailchimp form. How do I know if its mailchimp, or avada issue? I guess ill post everywhere?
