Your theme or plugin is modifying the PHP Mailer which is required for FLuent SMTP to work. If you are using any other SMTP plugin then deactivate it. If it’s coming with the theme or plugins then ask their support to provide a hook to disable modifying of PHP Mailer.

As you mentioned, Fluent SMTP test emails arrive so it’s working. Can you please check the FLuent SMTP log that whether the Fluent Forms notification email is logged or not? https://prnt.sc/1mzwnnz If it says failed then click on the eye icon and look for the technical information which will show you why it’s failed. https://prnt.sc/1mzxq5a
You can also check the submission log of the entries from Fluent Forms which will also show you why it’s failed. https://prnt.sc/1mzz4cc
After finding the issue which causes the notification not to be sent, we can provide a detailed solution.

Which version of PHP you are using? We found that the SES driver causing this issue if the PHP version is 8.
While we fix the issue on the next update, I recommend using PHP 7.X for now.Thanks

Which caching plugin you are using? Maybe it’s due to the caching issue, to make sure please deactivate the caching plugin for a moment and then test it then again you can activate it.

If you use the API using an email( [email protected] ) the form email always will be [email protected] because Gmail will force the sending name.
If you want to use something custom([email protected]) use Amazon SES or MailGun or other sending services instead of Gmail.

Which sending service you are using? Amazon SES or something else? The emails are delivering to Gmail inbox which means Fluent SMTP is functioning well. The issue is somewhere else.
It can be the sending service or Microsoft services(Outlook, Hotmail) itselfOutlook and Hotmail sometimes or always block unencrypted emails very strictly than Gmail. You need to make sure you are forcing encryption.
Also, verify your SPF and DKIM records with the domain if you are using a custom domain as a sending domain.

The error ‘SMTP connect() failed’ can be caused by many reasons, here are a few:
1: Wrong username and password are given in SMTP authentication.
2: SMTP port blocked in mail server firewalls.
3: Security restrictions by 3rd party servers such as Gmail or email apps.To check what’s causing the issue we need more information. We might need temporary access also later. Now it would be better if you open a thread on our private dedicated support channel.
