Thanks for the reply. I really appreciate it as this issue is really frustrating me.

Here is what I get.

For some reason, the redirect URL always is populated with index.php

It probably is a weird interaction with another plugin I have installed. It only is reproducible when I have per user enabled. I will I will keep that enabled and attempt to disable other plugins until I find the cause.
