@jamesfawcett no solution yet but from troubleshooting we discovered that the issue of displaying was with our theme. Perhaps try switching to default WordPress theme and see if that is the case for you as well.
Hi Shaylee, I will reach out via the support page. However, we had since downgraded to have our forms visible so the affected page link will show a working form.
