I think I was bitten by page caching. I set the HTTP header “Cache-Control: no-store” and my problem seems to have cleared up.

additional info: I have used the setting on the “advanced” panel to use the alternate session mechanism, but this only appeared to help. After a short while, the behavior manifest itself again.
