Turned on debug, not sure what would show up? and Where?

Nothing in the Salesforce debug logs
Tried disabling any new plugins installed
Tried creating a completely new form

Maybe something in the Salesforce account? Where would I start there?

Thanks @nickciske I’ll try your suggestions and let you know. I just recently jumped in on this issue. Apparently, it was working previously and stopped a few weeks ago.

I’ll report back once I try your suggestions.
