I solved the problem… ZIP code input does not respond if jQuery 2.2.2 library is being used for other modules.
Great plugin overall, but support could use some improvement.

I don’t have any sever side or caching plugin activated.
Maybe this provides more info, i am using Divi Theme and it functions with a file name (et-cache) in domain.com/wp-content
