I spent several hours over the weekend trying to debug this issue.
I’ll contact our hosting provider and report back.
I’m experiencing the exact same issue. I’ve deactivated all of my plugins and tested again but I’m still experiencing the same issue. I’ve also reverted to twentyeleven theme and the same issue persists.
It was either added later or I completely missed the ability to have an asterisk displayed next to the label if the field is required. Either way, thanks again.
I’m having the same issue. I ended up editing the form output code in the database.
