This almost works with the $200 Table Rate Shipping WooCommerce plugin (along with the right settings), but I still end up with Shipping #1 on the left and the right side squeezes both the name of the shipping class and price. I’d like to do away with Shipping #1 and simply show the shipping class name on the left and price on the right.

Just tried this plugin with WP 3.5 (no other plugin installed) and it seems to disable the “delete” links within the admin plugin page as well. Disabling the WP Mailto plugin is the solution for now.
