Just an update. So we cloned the site and tested it on a completely different server setup. We found that the error doesn’t occur on the different server setup.
So its not something to do with the plugins, theme or site configuration. But most likely something to do with the web server and its configuration.
Could you point us in the right direction? Thanks

So its not an issue of whether or not it adds to the library. The issue is that the image errors after adding and timeouts, then it eventually adds to the media library.
Are you getting this same issue, is probably a relevant question here.
