I have just put together a shortcode for getting the default Ultimate Member Password Update form any where on your site.

Perhaps you can try this plugin Login Widget With Shortcode
It has a shortcode
[forgot_password]which states “Use this shortcode [forgot_password] in your page to display the forgot password form.”I hope that helps.
This is a popular plugin that allows you to switch to a user account/ login as a user. Perhaps this may assist you. If it doesn’t, please let me know.
