I have the same issue using Attachments 3.3 and WordPress MU (with or without other plugins activated). If the plugin is activated per blog rather than across the whole network it gives errors when uploading / selecting files. Would love some help with this 🙂
If anyone is still looking at this issue – I simply changed the name and id of the submit button to anything but “submit” and it worked like a dream!
