Awesome! Thanks so much for looking into it.
I’ve used this plugin for over a year, and when I upgraded this time, the form on each item disappeared. I made no changes to the plugin. I have disabled all other plugins and it still doesn’t work.
I already tried that and it didn’t work…
WordPress 3.0.1.
