This plugin does EXACTLY what I’m looking for: http://ww.wp.xz.cn/extend/plugins/scategory-permalink/
Seems you just run your mouse up and down the category checkboxes for any post and choose “primary” on the one that you want to be the main one.
Then you use %scategory% instead of %category% in the permalink structure.
PERFECT!

Incidentally, had I changed from mydomain.com to blog.mydomain.com, would I have had to do anything differently? If so, which instructions would I have had to follow?

Worked perfectly, and it updated my permalinks without problem.
I wasn’t sure what was going to happen if I tried this myself–honestly, the documentation is not good on this subject.
