I did not find a solution. I instead deleted the plugin and went with a different one, WP Activity Log. It works.
And no cache system that I am aware of (and I should know). I double checked all the plugins, but again, with the troubleshooting mode thing, it was like Simple History was the only plugin installed, and it still wouldn’t display anything.
Today, I deleted the plugin folder via file manager to prevent the “deactivate” code from running. I then reinstalled from Add Plugins and activated. Same problem.
Then, I in stalled the ww.wp.xz.cn health and troubleshooting plugin, and clicked troubleshoot on Simple History, which is supposed to run the site for me with ONLY Simple History running, and still the problem.
It is very clearly logging the activity to the database, but refuses to display any. Could there be some issue with the format of time/date somewhere? I did change host servers a while back, and honestly, I don’t remember for sure if it was working before that or not.
Just updated to the new 3.0 release, and the problem still persists.
